چکیده

A 83-year-old woman underwent aortic valvular replacement with a bioprosthesis for symptomatic aortic stenosis. Anesthesia and surgery were uneventful. Unfractionated heparin (UFH) had been administered for cardiopulmonary bypass followed by anticoagulation on day 0 (surgery), and had been continued thereafter. Routine postoperative blood count showed an increase in platelet counts to 200 g/L on day 6. On the ninth day, the patient complained of paresthesia of the left foot. A computed tomography scan with intravenous radiocontrast material was performed. Thrombosis of both iliofemoral axes and a thrombus on the ascending aorta were found (Figure 1 and Figure 2). A blood count on day 9 revealed thrombopenia at 44 G/L; antiPF4 antibodies

Comparison between complications of vascular surgery procedures with and without aPPT assessment and protamine sulfate reversal

Abstract Background: Thrombosis will occur due to contact of blood with unepithelialized surfaces after vascular clamping and also blood stasis during vascular surgery. Heparin is administered to prevent thrombosis. The aPTT test is used for assurance of the anticoagulative effect of heparin. At the end of the operation heparin is neutralized by protamine sulfate in some centers. In this study...

Early post-operative thrombosis of the prosthetic mitral valve in patient with heparin-induced thrombocytopenia

Heparin-induced thrombocytopenia (HIT) is one of the most common immune-mediated adverse drug reactions, with frequencies as high as 2-3% for certain groups of post-cardiac surgery patients. We report on an 50-year-old woman with early post-operative thrombosis of the prosthetic mitral valve due to heparin-induced thrombocytopenia. Non-invasive imaging (two-dimensional transesophageal echocardi...

Lower extremity manifestations and treatment of heparin-induced thrombocytopenia syndromes: a cohort study.

Heparin-induced thrombocytopenia (HIT) and heparin-induced thrombocytopenia with thrombosis (HITT) syndromes are the result of an adverse reaction to heparin that results in a spectrum of laboratory and end-organ manifestations secondary to thrombosis of both arterial and venous small and large vessels. HITT most often manifests in the extremities as acral ischemia and necrosis, with a spectrum...

Thrombosis due to Heparin-induced Thrombocytopenia in Cardiac Surgery: is Fondaparinux an Effective Treatment?

Objective: To determine whether fondaparinux is effective in the treatment of thrombosis due to heparin-induced thrombocytopenia (HIT) in cardiac surgery we reported three cases successfully treated with fondaparinux and withdrawal of all heparins. Patients: All three patients were female and underwent heart valve replacement using cardiopulmonary bypass. Combined clinical criteria, "4T score" ...
